Important Information
New Patient Appointments: New patient appointments are currently being scheduled approximately 8 weeks in advance. Your membership officially begins on the date of your first appointment.
Expecting a New Baby? If you are enrolling an anticipated newborn, please complete enrollment 6–8 weeks before your due date to allow adequate time for the anticipated scheduling.
What Happens Next? Once we receive your enrollment, we will contact you within 24–48 business hours to schedule your appointment and send your enrollment packet.
Payment Information: Payment information is required to complete enrollment; however, you will not be charged during the online enrollment process. We understand this can be confusing, as payment information is collected upfront. Dates may not be appropriately reflected during the enrollment process.
Membership Fees: Your enrollment fee will be charged on the date of your (or your family member's) first appointment. Your monthly membership fee will not be charged until the end of your first month of membership.
